Drooping eyelids and eyelid surgery

Find out why excess skin and true eyelid drooping need different assessments and sometimes different treatments.

What it is

Two different problems can make an upper eyelid look heavy. Excess eyelid skin can hang over the lid crease. Ptosis means the eyelid edge itself sits lower than it should, often because the lifting mechanism has weakened. Brow position can also contribute. The right treatment starts with identifying which structures are responsible. 1 · 2

What you may notice first

The lids may feel heavy, look uneven, or block the upper part of your vision. Some people raise their eyebrows or tilt their head to see more comfortably. A change in eyelid appearance does not always mean surgery is needed. 1 · 3

How it is diagnosed

The examination assesses eyelid height, lifting strength, brow position, eye movements, and the eye surface. Photographs and visual-field testing may help document an effect on vision. New or changing ptosis sometimes needs testing for an underlying nerve or muscle condition. 2

Usual treatment options

Observation may be reasonable when the problem is mild. Upper blepharoplasty removes selected excess skin or tissue; ptosis repair adjusts the eyelid-lifting mechanism. Some acquired ptosis may respond temporarily to prescription drops, and underlying medical causes require appropriate treatment. Surgery can involve dry-eye symptoms, incomplete closure, asymmetry, scarring, bleeding, infection, or a need for further correction. 1 · 2

Beatific Vision evaluates upper eyelid concerns and offers upper blepharoplasty and ptosis repair when appropriate.

When to seek urgent care

A suddenly drooping eyelid needs immediate medical assessment. If it occurs with a severe headache, double vision, a change in pupil size, weakness, or trouble speaking, call 911 or go to an emergency department. Do not wait for a routine appointment. Sudden vision loss, severe pain, or rapidly increasing swelling after eyelid surgery also needs immediate attention. 4 · 1

Questions to ask

  • Is the problem excess skin, the eyelid muscle, my brow, or a combination?
  • Could treatment affect eye closure or dryness, and what improvement is realistic?
